Why do wall ties fail?

Installation faults
Even stainless ties can fail if they were installed incorrectly. Common faults include too few ties (especially around openings and at gable verges), ties that are too short for the cavity so they barely grip one leaf, ties sloping towards the inner leaf so water runs inside, mortar droppings bridging the cavity, and ties bent or cut to fit.
